Virtual Test Instructions
- Register for Test Session
- Performance Affidavit (Download PDF) Complete form and send to skokietestchair@gmail.com
- Release and Consent Form (Download PDF) Complete form and send to skokietestchair@gmail.com
- Email video (via Google Drive link or DropBox link) to skokietestchair@gmail.com
- If all contents are not received or are not in an appropriate format by the deadline, a late fee will be applied.
